I've completed it but seem to be missing an Achievement... i have Duke of Dementia, not Mania 
whats up with that? re-doing the game will take forever to get this achievement. so annoyed.
any ideas?

Have yet to buy the expansion, but will remember to do 2 saves at that point.

I played through the expansion with 2 characters - one went to Mania, one went to Dementia, thus unlocking both achievements.
However, it's not possible to get both on the same character if you didn't create a game save before you chose.
Anyway, if you really want that achievement, you can create a new character and go straight to Shivering Isles when you leave the sewers and you should be able to play through the questline even from Level 1.

yeah it's tricky this is one reason when I play a game I have many saves in my memory card.
