- Andy's show has been notable for featuring an all star cast of celebrity guests including Lil Jon, Paul Wall, Mike Jones, Nick Cannon, Biz Markie, Black Eyed Peas, the Ying Yang Twins, Snoop Dogg, Fat Joe, Juelz Santana, Rob Schneider, Carson Daly, Jimmy Kimmel, Seth Green, John Stamos, Vanessa Minnillo, Shaun White, and The All-American Rejects.

- Andy: I’ve been a hip hop head forever. So when it turned out that we were gonna start getting celebrity guests for every show, I wanted rap guys from the get-go. I just thought that even though not everybody is hip hop (on the show), about 80 percent are. I just think that it’s such a weird wacky comedy-based world, and then you bring in those rappers who are typically all about money, cars and girls and we put them in these wacky situations, and we don’t really treat them like celebrities. Like Lil’ Jon was the first, and instead of treating him like Lil’ Jon with the girls and stuff, we just had him doing some stupid sh*t with me so we just like the contrast of taking these rap guys and putting them in wacky situations.
- Andy (about being a white hip hop fan): I’ve been into it (hip hop) my whole life, but it's different from the white kids who 'dress the part,' and their slang is obviously fake and all that. When these kids pose, it sounds so fake when every other word is like, 'Yo, yo yo, and knowhutimsayin?' I might say some shit when I’m around my boys and stuff, but I hate the ones who give white kids who listen to rap a bad name.
- Andy (asked if his show appeals more to kids or adults): The show is like anywhere from eight to 25. I don’t know too many people over 30 who like it, but I definitely have some middle-aged people who tell me they watch the show, but I definately think it’s for a younger audience. Some of the stuff I used to do before I got the show was for an older audience.
- Andy (asked to describe his show): I would say it’s intensely bad public access television.
- Andy:: 50 Cent’s bodyguards kicked me out of a green room once. I was supposed to be there. I was in Cancun for spring break shooting a show and they were like, 'Yo, bounce. This is reserved.' I said, 'Alright, but next time I see you it’s on.'
- Andy: I’m a huge fan of Big L. He is my favorite rapper. I like Styles P from the Lox. I like Game’s '300 Bars' a lot too.
- When asked his age in an interview, Andy responded, "somewhere between 10 and 30".

- Andy created a new rap for the end credits to the second season of his show and no longer uses Green Day's American Idiot.
- The popular comedian and host, Jimmy Kimmel, helped Andy get his start by recruiting him for regular appearances on his late night talk show, Jimmy Kimmel Live, during the years of 2003–2004.
- Andy has a talent for freestyle rapping. On Last Call with Carson Daly, he rhymed about chocolate at the request of an audience member.
- Andy credits Jimmy Kimmel with discovering him, and acknowledged this with a Pinnochio segment during season two of The Andy Milonakis Show. In the sketch, Jimmy Kimmel (acting as Gepetto), crafts Andy from a small piece of wood, places a bag of frozen peas on Andy's head and exclaims "You will be my son, Andy Milonnochio!"
- Andy has a congenital growth-hormone condition that gives him the outward appearance and voice of an adolescent boy, when he is in fact a grown man. Due to his disorder, earlier in life he was subjected to frequent prejudice, but Andy has strong hopes for biology to one day provide a cure.

- Andy uses his condition as an asset rather than a handicap, playing the role of a pre-pubescent boy on his sketch comedy program, The Andy Milonakis Show. The first season aired on MTV, and the show currently airs on MTV2.
